Services and Fees

 

Payment is due at the time of service. Accepted forms of payment include Health Savings Account (HSA) and Flexible Spending Account (FSA) funds, most major credit cards, checks, and cash. While Square One does not have contracts with any insurance companies, you may submit a claim directly to your insurance company for possible reimbursement through out-of-network benefits. We provide the necessary information for your claim, including our federal tax identification number, diagnosis (DSM/ICD) codes, and procedure (CPT) codes. Our doctors will provide signatures or other documentation on a requested basis.

MEDICAL:
DEVELOPMENTAL PEDIATRICIAN 

Pediatrician Hourly Rate:
    $250/hour

Behavioral-Developmental Evaluation (includes 1 1/2 hour comprehensive clinical interview with parents followed by a 30-45 minute neurological and physical evaluation of the patient. Fee includes physician’s written evaluation.):
    $500; duration 2 hours

Follow-up Evaluation (when necessary):
    $250; duration 1 hour

Consultation Appointment with Developmental Pediatrician (includes explanation of evaluation findings, diagnoses, recommendations, and treatment options. This appointment should be scheduled for two weeks following the evaluation appointment. You will receive the written comprehensive report approximately two weeks after your consultation appointment.):
    $150/one hour session

Medication Check:
    $187.50; duration 45 minutes plus write up and periodic phone consults >10 minutes

CHILD PSYCHIATRIST (not currently offering psychiatric services)
Psychiatrist Hourly Rate: $250/hour

New Psychiatric Evaluation: 1 1/2 hours

Medication Check: 45 minutes

LICENSED PSYCHOLOGICAL:
CLINICAL PSYCHOLOGIST, PSYCHOLOGICAL ASSOCIATE, CLINICAL SOCIAL WORKER

Psychological, Psychoeducational, Emotional, and/or Behavioral Evaluation (includes clinical interview with parents, clinician’s scoring, and written evaluation) varies according to age:
     $450 Children 1 to 5 years; duration 2 1/2 hours
     $525 Children 5-6 years; duration 3 hours
     $600 Children, Adolescents, Young Adults 7-21 years; duration 3 1/2 hours

Consultation with Clinical Psychologist or Psychological Associate (includes explanation of evaluation findings, test scores, diagnoses, recommendations, and treatment options. This appointment should be scheduled for two weeks following the evaluation appointment. You will receive the written comprehensive report approximately two weeks after your consultation appointment.): 
    $150/one hour session

IQ Testing (includes immediate feedback, clinician’s scoring, and written evaluation):
     $300; duration 1 1/2 - 2 hours

Individual/Family/Play Therapy:
    Psychologist: $110/hour
    Social Worker: $95/hour

Initial Intake Evaluation:
     Psychologist: $150/hour
     Social Worker: $115/hour

Autism Diagnostic Observation Schedule (ADOS):
    $250/session

Parenting and Social Skills Groups (group durations vary):
    $50 for each 60 minute session
    $80 for each 90 minute session

SPEECH/LANGUAGE PATHOLOGIST
New Evaluation: $100/hour
     Children 1 to 5 years: duration 2 hours
     Children 5 to 10 years: duration 2 1/2 hours
     Children and Adolescents 10 years and older: duration 3 hours

Speech-Language Therapy:
     $85/hour

TEAM EVALUATION
Our experienced staff work as a team to provide multidisciplinary in-depth assessments and to recommend treatment options. Each specialist is also available to see patients individually as outlined above.

New Developmental Evaluation (all ages): $500, includes 1 1/2 hour clinical interview with parents followed by a neurological and physical evaluation of the patient.

This appointment should be scheduled for two weeks following the evaluation appointment. You will receive the written comprehensive report approximately two weeks after your consultation appointment.):
    $150/one hour session

For example, the fee for a team evaluation for a 7 year old youngster would be $500 for the medical evaluation, $525 for the psychological examination, and $150 for the parents-only consultation for a total of $1175. Fees for children under age 7 are outlined as above.

PHONE CONSULTATIONS
While there is no charge for brief email or phone questions, lengthy calls or emails (over ten minutes) will be billed at the physicians'/clinicians' respective hourly rate.

MEDICAL RECORDS REQUESTS
Patients/guardians are entitled to one free copy of the medical record. As required by Kentucky law KRS 422.317, “A copying fee, not to exceed one dollar ($1) per page, may be charged by the health care provider for furnishing a second copy of the patient’s medical record upon request either by the patient or the patient’s attorney or the patient’s authorized representative.”

LITIGIOUS CONSULTATIONS
